Shropshire Council launches Shropshire Wellbeing and Navigation Network for Adults

New service aims to boost independence, wellbeing and community connection across the county

Shropshire Council and partners Shropshire Integrated Care Board (ICB) are delighted to announce the launch of the Shropshire Wellbeing and Navigation Network (SWANN), a new preventative support service for adults aged 18 and over living in Shropshire. Designed to help residents stay well, maintain their independence, and feel connected to their local communities, this innovative service forms part of a wider commitment to strengthen adult preventative health and social care across the county.

SWANN provides timely support for adults before challenges escalate, reducing the need for crisis or statutory services. It is particularly aimed at individuals who may be finding it harder to manage at home, experiencing low-level mental health difficulties, feeling socially isolated, recovering after illness or hospital discharge, living with long-term conditions, or unpaid carers needing additional help.

Support is delivered through three themed areas: Supported Hospital Discharge (British Red Cross), Community-led Mental Health Outreach (Shropshire Mental Health Support and Community Resource), and Wellbeing and Independence (Live Well Shropshire). Services include practical and emotional support, peer groups, befriending, physical activities, digital inclusion, and guidance to build confidence and resilience.

Councillor Ruth Houghton, Portfolio Holder for Adult Social Care, said:

‘We are pleased to launch the new Shropshire Wellbeing and Navigation Network, which will make a real and positive difference to adults across the county. This service means that people can live the lives they choose, knowing that help and support is available when it’s needed. By working with our partners, we are strengthening community-led support and ensuring that people have access to the right services before challenges escalate.’

The service is preventative and time-limited, and does not replace statutory care, clinical health services, or emergency/crisis interventions. If urgent or specialist help is required, professionals should follow existing referral pathways, including emergency services or NHS urgent mental health support.
