Chelsea earned a 4-1 victory against Spanish side Real Betis in the final on Wednesday night, despite having trailed 1-0 at half-time.
Cole Palmer set up goals for Enzo Fernandez and Nicolas Jackson in a six-minute second half spell to turn the match into the Blues’ favour.
Jadon Sancho and Caicedo struck late on to seal victory for Enzo Maresca’s side.
Chelsea stars celebrated on the pitch at the Stadion Wroclaw, joined by family and friends.
Fernandez was joined on the pitch by his childhood sweetheart Valentina Cervantes, as well as their daughter and son, with the World Cup winning midfielder adding ‘one more’ as a caption following his latest final success.

Following the on-pitch trophy lift, Chelsea stars moved onto a private event in the Polish city, but gave fans insight into the celebrations through social media.
Trevoh Chalobah shared a video in the early hours of the morning of Caicedo, with the Ecuadorian midfielder having taken a microphone at the event.
The 23-year-old, still wearing his kit from the match and draped in an Ecuador flag, was heard signing as Chelsea goals from the tournament were played on a screen in the background.
Neto and goalkeeper Filip Jorgensen, while not appearing with the microphone, sang along to Uptown Funk in another clip from the post-match party.
Chelsea defender Marc Cucurella joined a photo with forward Marc Guiu from the event.
While Guiu could be seen with the Catalan flag wrapped around his waist, Cucurella donned ski goggles while wearing his kit from the final.

The celebrations also featured suspended winger Mykhailo Mudryk, who had earlier watched the showpiece from the stands.
Mudryk was among those in attendance at in the Polish city Wroclaw, with the winger travelling separately from the team for the match.
The Ukrainian had been pictured in the build-up to the match in a steakhouse, with a fan taking a photo with the 24-year-old, who was pictured in a club gilet.
Mail Sport reported that Mudryk would watch the final at Stadion Wroclaw, with Chelsea grateful for his contribution in the early stages of the competition.
Mudryk has not appeared since November 28, when he scored in a 2-0 win at Heidenheim in this competition in the league phase. The Ukrainian was then placed on a temporary suspension after failing a doping test.
Mudryk shared a photo on social media from the stands, with the Ukrainian watching on as his team-mates lifted the Conference League trophy.
He later joined the Blues squad for their post-match celebrations, with midfielder Enzo Fernandez posting a picture alongside Mudryk on Instagram.
The 24-year-old was pictured wearing a Conference League medal as he posed next to Fernandez.

It is unclear whether the medal worn by Mudryk was his own or belonged to Fernandez, with the midfielder pictured without his prize.
Mail Sport reported pre-match Mudryk was expected to receive one of the 50 gold medals given by UEFA if Chelsea triumphed.
Mudryk had contributed in the early stages of Chelsea’s run to the Conference League title, scoring three goals in six appearances.
